Purpose
This form is required for participation in workplace wellbeing sessions, including (but not limited to) holistic therapies (including massage & Reflexology), Pilates, and soundbaths. It is designed to ensure your safety and to meet insurance requirements. This form applies to all wellbeing sessions booked through your company alone.
You must complete and resubmit this form if there are any changes to your health.
All information provided will be treated confidentially and in accordance with data protection regulations. Your details will not be shared with third parties.

By signing this electronic form, you confirm that you:

Agree to take part in this wellbeing session at your workplace.

Understand it is your responsibility to check with your GP or medical professional if this session is suitable for you.

Have or will informed the therapist of any relevant health conditions, including injuries, illness, pregnancy, recent operations, medical treatments (including injectables), allergies or other relevant medical health conditions in advance of the session starting.

You are responsible for bringing any necessary medication (e.g. EpiPens, inhalers) to sessions and ensuring it is accessible. The facilitator and company are not responsible for providing or administering medication.

Will not attend the session if you have a contagious illness.

Understand that sessions should not take place within 48 hours of a vaccination or withing 4 weeks of Botox injections or and other invasive aesthetics.

Acknowledge that all wellbeing therapies carries some level of risk and agree not to hold the therapist liable for any adverse reactions.
The therapist (Sanchia, Head to Toe Harmony), accepts responsibility for any injury caused by faulty equipment provided by them.
